The undersigned confirms that the information provided in this application is true, accurate, and complete. By signing this
application, the undersigned authorizes Groupe SFA / Affiliated Financial Services Inc., its affiliates, service providers, and any
third-party financial institutions involved in the financing request (collectively referred to as “AFS”) to collect, use, retain, and
disclose personal and/or commercial information for the following purposes (the “Purpose”):
- To verify the undersigned’s identity and evaluate creditworthiness
- To assess eligibility and determine terms for credit or leasing
- To comply with applicable legal and regulatory obligations
- To manage the client relationship, including communications related to the application or future financing
StriveX Financial Services is authorized to obtain personal or commercial information from, and disclose it to, credit
bureaus, lenders, credit agencies, and other third parties with whom it has a financial or contractual relationship, as required
to fulfill the Purpose.
The undersigned acknowledges that a Credit Application File will be created and stored on secure servers located in Quebec
and/or the United States, and will only be accessed by authorized personnel. Whether or not credit is granted, the
undersigned has the right to access, correct, or request deletion of their personal information, subject to legal requirements,
by contacting StriveX Financial Services at info@strivexfinancial.com
StriveX Financial Services may assign or transfer this application in whole or in part, and may disclose personal or financial
information in accordance with such assignment, as permitted by law.
